Health History Form 
Paper needs to be sign in one spot by parent.  Student -athlete cannot have physical without this sheet being filled out.
Risk of Injury Statement 
Parent and student-athlete must sign this also.  It must be turned in with the Health History Form.
Training Rules Sheet 
This must be signed by Student-Athlete and Parent.  Student-Athlete and Parent must read before signing.  This paper must be turned in also at time other papers are turned into the nurse.
